Titanium Dental Implant Removal & Biological Implant Replacement

For many patients, titanium dental implants function well and remain an excellent solution.
However, a growing group of patients experience unexplained symptoms, biological intolerance, or local complications that prompt a deeper evaluation of whether titanium is the right material for their body.

My approach is not anti-implant—it is pro-biology, pro-function, and pro-patient.

Who may benefit from titanium implant removal?

Titanium implant removal is not for everyone, but it may be appropriate for patients who experience:

  • Persistent inflammation or non-healing around implants

  • Recurrent peri-implantitis despite good oral hygiene

  • Facial pain, jaw discomfort, or unexplained bite instability

  • Chronic fatigue, brain fog, headaches, or systemic symptoms without a clear cause

  • Known metal sensitivities or autoimmune conditions

  • A strong preference for metal-free, biologically inert materials

Importantly, some patients feel “not quite right” after implant placement, even when X-rays appear acceptable. These cases deserve careful, respectful investigation—not dismissal.

What are the concerns with titanium?

Titanium is widely used because it is strong and integrates well with bone.
However, from a biological perspective, it is still a metal, and metals can behave differently in different bodies.

Potential concerns include:
  • Titanium particle release at the implant–bone interface

  • Immune sensitisation or inflammatory response in susceptible individuals

  • Galvanic reactions when different metals are present in the mouth

  • Soft tissue inflammation that does not fully resolve

These reactions are uncommon—but when they occur, simply “changing the crown” is often not enough.

Titanium vs ceramic implants: a biological comparison

Titanium implants

  • Metal-based

  • Excellent mechanical strength

  • Long track record

  • May release microscopic particles over time

  • Can provoke immune or inflammatory responses in sensitive patients

Ceramic (zirconia) implants

  • Metal-free and highly biocompatible

  • Chemically inert and electrically neutral

  • Reduced plaque attraction

  • Excellent soft-tissue response

  • Favoured in biologically focused dentistry

Ceramic implants are not a trend—they are a material choice rooted in tissue behaviour, immunology, and long-term balance.

Health and medical considerations

In selected patients, metal implants may be associated with:

  • Chronic inflammatory load

  • Immune activation or sensitisation

  • Worsening of underlying autoimmune or inflammatory conditions

For this reason, I assess the whole patient, not just the implant site.

Pre-treatment assessment: what patients need to prepare

Before considering implant removal and replacement, I request targeted medical and biological information, including:

  • Vitamin D3 levels – essential for bone healing and immune regulation

  • LDL and metabolic markers – indicators of systemic inflammatory state

  • Recent blood tests assessing immune activity and inflammatory markers

  • Where relevant, allergy or sensitisation screening

My standard biological protocol includes:
  1. Biological Assessment, Diagnosis and Treatment Plan of Biological Health

  2. Correction of Careful, controlled removal of the titanium implant. 

  3. Implant removal is done atraumatically and methodically, preserving bone wherever possible.

  4. Thorough decontamination of the site

  5. Ozone therapy to sterilise and detoxify the area

  6. Placement of platelet-rich fibrin (PRF) to promote healing and regeneration

  7. Immediate or staged planning for replacement, depending on biology and stability

Bone preservation and biological cleanliness are priorities at every step.

Same-day replacement with ceramic implants

Where conditions allow, I may perform:

  • Same-day titanium removal and ceramic implant placement

  • Same-day teeth using ceramic implants in carefully selected cases

This is determined by:

  • Bone quality and stability

  • Infection status

  • Bite forces

  • Overall systemic health

When appropriate, this allows treatment to be completed in one session or two, reducing downtime and overall treatment burden.

Comfort-first philosophy

Comfort is not optional—it is central to my approach.

Procedures can be carried out under:

  • Local anaesthesia

  • IV sedation

  • General anaesthesia (where indicated)

The aim is to complete treatment efficiently, comfortably, and safely, in the shortest time frame possible—without compromising biological principles.

I believe dentistry should be biologically compatible, functionally sound, and system-aware.

I have worked extensively with ceramic implants for many years, and my focus has always been on:

  • Reducing inflammatory burden

  • Supporting natural healing

  • Respecting the immune system

  • Delivering long-term, stable outcomes

Implant removal is never undertaken lightly—but when it is the right decision, it can be genuinely life-changing for the right patient.
